Acknowledgment of risk and assumption of riskI understand that athletic training and the use of Armed Sports Program facilities and equipment involve inherent, unanticipated and unknown risks that may cause injury or death. I voluntarily assume these risks, including risks arising from equipment, the actions of others, and my own actions or misuse.Release, covenant and promise not to sueIn consideration of being permitted to use the Armed Sports Program facilities, services and equipment, I release and discharge the facility, its successors, assigns, officers, directors, agents and employees from claims and liability arising from participation, and I agree not to bring an action against those released parties to the fullest extent permitted by law.Indemnification agreementI agree to indemnify and hold harmless Armed Sports Program, its successors, assigns, officers, directors, agents and employees from claims, demands, liability or judgments arising from my participation or use of its facilities, services and equipment.Parent, guardian or conservator agreementIf signing for a child or ward, I agree to indemnify and hold harmless Armed Sports Program and its successors, assigns, officers, directors, agents and employees from claims made by or on behalf of the child or ward arising from participation or use of the facilities and equipment.Medical acknowledgment and releaseI certify that the participant has no known medical condition that would preclude participation and that the medical information provided is accurate to the best of my knowledge. Participation is voluntary. I understand Armed Sports Performance Training - Humble does not diagnose or treat medical conditions, makes no guarantee of results, and is not responsible for personal-property loss. I release Armed Sports Performance Training - Humble and its affiliates, employees, agents, representatives, successors and assigns from claims arising from participation, including equipment use or malfunction and slips or falls, to the fullest extent permitted by law.ConfirmationI have carefully read this waiver and release, understand that it includes a release of liability, and voluntarily agree to its terms.
